## The field she had never seen

Divya had been at the health-tech company in Ahmedabad for nine days when she was assigned to the ABDM integration sprint. The company built a hospital management system — HMS — for small and mid-sized hospitals across Gujarat. Patient registration, appointment scheduling, billing, lab-report management. Divya had built web applications for two years. She had never built anything for healthcare.

The sprint planning ticket said: "Integrate ABHA-based health record linking. Patient should be able to scan ABHA QR at OPD registration. System must register as HIP with ABDM sandbox. FHIR-compliant discharge summary to be pushed to patient's linked records with consent."

She understood every English word in the ticket. She could not connect a single one of them to anything she knew. ABHA. HIP. FHIR. Consent flow. Linked records. She opened the ABDM developer documentation and found a system with more moving parts than anything she had worked with before — not because the technology was exotic, but because the domain underneath it was. The technology was APIs, JSON, OAuth. The domain was clinical data, patient consent, regulatory compliance, and the institutional reality of how a two-hundred-bed hospital in Mehsana actually works.

She spent her first afternoon reading documentation. By evening, she had a vocabulary problem, an actor problem, a money problem, a rules problem, and a failure-modes problem — all at once, all unfamiliar. She had, in other words, exactly the situation the five-thing map was built for.

This chapter draws that map for Indian healthcare.

## 1. Vocabulary — the words that run Indian healthcare systems

Healthcare has one of the densest specialised vocabularies of any domain, and the digital layer adds its own. Here are the terms you cannot function without if you are building health-tech in India.
